Enrollment

• Ages of admission are infants through five years of age.
• Applications are accepted subject to availability of space and accommodations.
• The following forms are required by the State of Florida and need to be completed before placement.
• Registration form signed by a parent or guardian, which includes the date of last physical examination.
• Complete record of immunizations.
• Written consent for child to receive medication.
• Signed polices and procedures agreement.
• A signed fee and payment plan (parent/provider contract) with a copy to parent.
• At least two phone numbers at which parents can be reached in case of emergency.
• Parents or guardians who take an active roll in their child’s up bringing are required to participate in a verbal orientations with the director before your child’s first day of school. This includes meeting all the staff of Building Blocks and visiting the child’s classroom.
